The life of a patient with penetrating cardiac injury is saved
A 49-year-old man was taken to Erebouni MC by ambulance, in extremely serious condition with a stab wound of the left side of the chest. Before arriving at the medical center, cardiopulmonary resuscitation was already performed, the patient was given artificial respiration. In MC Erebouni, the patient was urgently examined, as a result of which it turned out that the left side of the chest-the lungs, pericardium and heart was damaged as a result of a blow with a piercing and cutting tool. During the CT exam, hemopericardium, possible cardiac tamponade, hemopneumothorax were diagnosed. The patient was urgently taken to the operating room for vital indications. The surgery was started by thoracic surgeon Gor Melikyan and general surgeon Artak Vardanyan, anesthesiologist Artur Hovhannisyan. First, a left thoracotomy, pericardotomy, and removal of the cardiac tamponade were performed. In addition, a large damage to the apex of the left ventricle of the heart was found. Taking into account the severity of the patient and a complex surgical case, the surgery was continued with the direct participation and guidance of Dr. Ara Vardanyan, Deputy Director for Surgery of Erebouni MC. The case was special and remarkable, because there were changes in the heart muscle, which could be a consequence of previous myocarditis. As a result of the surgery, it was possible to sew the heart muscle using pericardial flaps and pericardial fat tissue.During a complex surgery, the patient's life was saved. In general, left thoracotomy, pericardiotomy, emptying of the cardiac tamponade, suturing of the injury of the heart muscle with the use of pericardial adipose tissue, suturing of a through lung injury, pericardial hemostasis, treatment of the pericardium and pleural cavity, drainage of the pleural cavity vein were performed. After the surgery, the patient received intensive medical treatment. Two weeks later, the man was discharged from the hospital.
