Urgent cardiology department with intensive care unit

General characteristics

To the treatment of acute coronary syndrome and acute myocardial infarction in the first hours of its occurrence is given special attention in all over the world, because a timely and highly qualified specialized care in the first hours of the disease is crucial in saving lives of patients.


Erebouni Medical Center: Cardiology & Cardiac Surgery Clinic: Urgent cardiology department with intensive care unitDepartment of urgent cardiology had been created to provide highly skilled, specialized care for patients with heart disease and consists of intensive care unit (ICU), Semi-Intensive Care Unit (the "second stage") and of the ward.


The close collaboration of the specialists of the Departments of Cardiology, Angiography and Endovascular Surgery, Cardiac surgery and Arrhythmology allows restoring patency of the artery and stopping the progression of myocardial infarction in the first hours of acute myocardial infarction.


This contributes to a significant reduction in percentage of patients with disability.


The main objective of the urgent cardiology department is the treatment of acute myocardial infarction and acute coronary syndrome.


The ICU receives patients with acute myocardial infarction, unstable angina, acute disorders of heart rhythm and conduction, congestive heart failure and other life-threatening abnormalities of the cardiovascular system. The department also provides treatment of the patients with chronic ischemic heart disease, hypertension, chronic heart failure, myocardial disease and pericardium, endocarditis, and other cardiac pathologies.


Department provides in-patients treatment for around 2500 – 3000 patients per year (among which 1000 – 1100 with acute myocardium infarction) and out-patients treatment for about 5000 – 6000 patients.

The Department is equipped with all necessary modern equipment. There are invasive and non-invasive bedside monitors, electrocardiographs, echocardiographs, treadmills, Holter-monitors. The department employs 20 highly skilled cardiologists, intensive care specialists who have been trained and specialized in leading foreign clinics. Treatment is carried out in accordance with the recommendations of the European Association of Cardiology and complies with international standards.

There are about 10-15 doctors per year who passing their residency on this clinical base. The results of conducted by department research are published in scientific cardiology journals of United States, Europe, Russia and presented as  scientific reports at the annual congress of the European Association of Cardiology (ESC) in 2006-2014, American and other cardiology congresses in 2009-2013. The physicians of the department involved in international multicenter randomized trials organized by major pharmaceutical companies and organizations.

 
Equipping

Department of urgent cardiology is equipped with all necessary modern equipment for diagnostics and treatment of heart diseases, in particular:

 

  • Monitors for intensive monitoring,
  • Electrocardiographs,
  • Temporary pacemaker,
  • Echocardiograph,
  • Holter -monitors,
  • Treadmills,
  • Pulse – oximeters,
  • Express – laboratory.

Over whole territory of the department and ICU a 24 hours videomonitoring and control are carried out.

 
Main activities

In the Department of Urgent Cardiology and ICU the medical care is performed on the basis of the determined international standards to the patients with the following diseases:

 

  • Chronic ischemic heart disease at the stage of its clinical worsening or at the stage of preparing for angiography or endovascular intervention, 

  • Acute coronary syndrome (with or without elevation of ST segment; acute myocardial infarction and instable angina); 

  • Acute myocardial infarction of the right ventricle;

  • Symptomatic arterial hypertension, which required further diagnosis and hospital stay;

  • Arterial hypertension, resistant to the therapy;

  • Acute and chronic myocarditis at the acute stage;

  • Cardiomyopathy, which requires the implementation of diagnostics and therapeutic procedures with hospital stay;

  • Hypertensive crisis complicated by acute heart failure, arrhythmias, and cardiac conduction, encephalopathy, ongoing epistaxis;

  • Pulmonary hypertension during the stage of decompensation or in case of necessity to perform additional diagnostic procedures with hospital stay

  • Cardiac rhythm and conduction disturbance, which require the implementation of diagnostic and therapeutic procedures with hospital stay;

  • Chronic heart failure in the stage of decompensation;

  • Pericarditis;

  • Infective endocarditis,

  • Pulmonary embolism;

  • Cardiac rhythm and conduction disturbance, accompanied by unstable hemodynamics and (or) the progression of coronary artery disease, heart and (or) cerebral vascular failure;

  • Acute heart failure (cardiogenic shock, cardiac asthma, pulmonary edema, circulatory collapse);

  • Syncopal condition of arrhythmic, vascular or obscure geneses; other forms of acute cardiac failure;

  • Dissecting aortic aneurysm.

 

The Department carries out the primary in-patient reanimation and treatment of patients with cardiovascular diseases and organizes consultation assistance to the physicians of other departments of the center in the problems of urgent cardiac care, treatment of cardio-vascular diseases and prevention of complications.
